上海市精神卫生中心特色学科建设
———精神医学精准检验学科
基于“精准医疗”的理念和临床需求，上海市精神卫生中心检验科开展精神医学精准检验学科建设。建设目标是协创精神疾病精准用药和个体化治疗，支撑精神疾病早期筛查和精准诊断，建设精神疾病精准检验医学示范体，达到全国一流精神科精准检验学科水平，成为上海市和全国同类医院精准检验特色学科和示范基地。建设内容包括实现精神疾病精准用药和个体化治疗、支撑精神疾病早期筛查和精准诊断以及牵头建设精神医学精准检验示范体等三个方面。
在实现精神疾病精准用药和个体化治疗方面，检验科积极进行治疗药物浓度监测平台、药物浓度监测第三方质控平台和药物基因组学平台建设。根据欧洲版《AGNP精神科治疗药物监测共识指南：2017》率先将治疗药物浓度监测按等级划分，率先规范推广精神科药物治疗参考浓度和警戒浓度；与上海市临检中心合作，开发精神科药物质控品并协助全市多家精神专科医院开展精神科药物监测的室内质控和室间质控；首次联合全市多家开展精神科药物治疗监测的专科医院，规范精神科治疗药物监测方法的一致性和同质性，实现同类同级医院治疗药物监测结果互认；建设周期内拓展推广药物基因组学，指导临床个体化用药。在积极支撑精神疾病早期筛查和精准诊断方面，认真进行精神疾病相关生物标记物研究平台、肠道微生态研究平台和诊断标记物转化平台建设，团队成员积极进行相关科学研究，从自闭症标志物研究、阿尔茨海默病与肠道菌群变化的联系等方面进行建设。在牵头建设精神医学精准检验示范体方面，进行精准检验多中心联合及检验数据整合信息共享平台的建设，依托全国多家同行业医院从CNAS 15189实验室认可、继续教育、学术、科研、教学培训等方面进行合作交流，建立精准检验的实验室诊断体系，为实现个体化医疗奠定基础。
同时，检验科积极加强团队建设，从技术、质量和科教等三个管理层方面进行建设。检验科顺利通过中国合格评定国家认可委员会CNAS 15189医学实验室认可的评审及复审，挂牌国家重点研发计划精神医疗集成应用示范体系核心单位和国家973计划临床研究实验室。同时加强科内后备学科带头人、科研和技术骨干等人才的培养，完善人才梯队。团队成员积极申请多项课题，现承担吴阶平基金课题、上海市卫计委中医药课题等，发表多篇中英文论文，主编及参编《临床检验一万个为什么》等10余部专业书籍。在学术方面，科室主任担任多项学会任职，团队成员积极参加学术会议，并积极承办多项国家级继续教育，参加了多种继续教育培训班。在教学方面，与商丘医专签署实习带教合作备忘录，并进行国内多家精神专科医院的进修见习带教工作。
Construction of Characteristic Subjects of Shanghai Mental Health Center
———Psychiatry Precision Medical Technology
Based on the concept of “precision medicine” and clinical needs, department of medical laboratory in Shanghai Mental Health Center carries out the construction of psychiatry precision medical technology. The goal of the project is to create the precise medication and individualized treatment for mental illness, support the early screening and accurate diagnosis of mental illness, and build the model for the precision medical technology of mental diseases. It will reach the national first-class level in precision medical technology and become the featured disciplines and demonstration bases in Shanghai and the whole country. The construction includes three aspects: achieving accurate medication and individualized treatment for mental illness, supporting early screening and accurate diagnosis of mental illness, and building the model for the precision medical technology of mental diseases.
In the implementation of accurate medication and individualized treatment for mental illness, the department of medical laboratory is actively conducting the construction of drug concentration monitoring platform, the third-party quality control platform for drug concentration monitoring and the construction of pharmacogenomics platform. According to "Consensus Guidelines for Therapeutic Drug Monitoring in Neuropsychopharmacology: update 2017", dividing the level of the therapeutic drug monitoring, taking the lead in standardizing the conception of psychiatric drug treatment reference concentration and alert concentration; cooperating with Shanghai Center for Clinical Laboratory to develop the quality control products for psychiatric drugs and assisting many psychiatric hospitals in Shanghai to carry out the IQC and EQA. Uniting lots of psychiatric hospitals in Shanghai to regulate the consistency of monitoring methods for psychiatric drugs and achieve mutual recognition of the results in similar hospitals of the same class; expanding and promoting the development of pharmacogenomics to guide the clinical individualized medication. In the aspect of supporting early screening and accurate diagnosis of mental illness, carrying out the construction of for the mental disease biomarkers platform, the intestinal microecological research platform and the diagnostic marker conversion platform. The team members actively carry out relevant scientific research for the biomarkers of autism, the association between Alzheimer's disease and intestinal microbiota. In the aspect of building the model for the precision medical technology of mental diseases, conducting the construction of precision medical technology union and data integration and sharing platform, establishing the laboratory diagnostic system for precision medical technology in many aspects, such as CNAS ISO 15189 laboratory accreditation, continuing education, academic, scientific research, teaching and exchange, and laying the foundation for individualized medical treatment.
At the same time, the department of medical laboratory is strengthening the team building actively from three management levels: technology, quality and scientific research. The department of medical laboratory has passed the accreditation and review of the CNAS ISO 15189, and been the core unit of the National Key Research and Development Program for Mental Health Integrated Application Demonstration System and the National 973 Program Clinical Research Laboratory. At the same time, training the talents in scientific research and technology, and improving the level of the talent team. We have many science projects, such as the projects from Wu Jieping's fund and the Shanghai Municipal Health Commission. We have published many papers in English and Chinese, and edited and participated the writing of more than 10 professional books. In terms of academics, the director of the department has served for more than 20 academic institutions. The team members have participated in the academic conferences actively, and hosted a number of national-level continuing education, and participated in a variety of continuing education training classes. We have signed a memorandum of cooperation with Shangqiu Medical College, and conducted training sessions for a number of psychiatric hospitals in China.
