上海市第六人民医院药学部

上海交通大学医学院附属第六人民医院(简称六院)药学部是上海市首批临床药学重点专科,上海交通大学医学院药学专业博士点和上海中医药大学中药学专业博士点,设有临床药学教研室。是中国药学会药学服务专委会、中国药师协会药品临床评价专委会、中华医学会临床药学专科分会、上海市医院协会临床药事管理专委会、上海市药学会医院药学专委会、上海市医学会临床药学分会的主委和副主委单位。现有药学专业技术人员182人,其中主任药师4人,副主任药师12人,包括Pharm.D在内的博士14人。医院“十四五”及2035规划着力打造“金字塔式”学科体系,将药学部列为“四大高原学科”之一。

六院药学部设立门诊西药房和草药房、急诊药房、发热药房、儿科药房、住院药房、静脉用药调配中心、小洋山药房、药品采购与药品政策研究部、制剂室、质检室、临床药学室、审方中心、治疗药物监测及个体化用药研究室、临床药学教研室等20余个部门。主要负责全院药事管理与药物治疗学委员会的日常工作;推动基本药物、集采药品和国谈药品等政策落地,保障药品供应;牵头落实和推进全院抗菌药物管理、精神麻醉类药品管理、药品不良反应监测、处方点评、药品质量与药事管理质控、药品议价和药品评价保障等药事核心工作;同时进行药学成果转化。经过长期积累,在药事管理、临床药学、教学、学术研究等领域硕果累累。获“2023年度上海市巾帼文明岗”荣誉称号,门急诊中西药房在2019年被评为一星级“全国青年文明号”,工会在2023年被评为“全国教科文卫体系统模范职工小家”。

药事管理

六院药学部结合我院实际情况,快速推进两院一体化建设,实现管理制度、药品目录、质控管理标准等多项工作内容的一体化,扎实开展了“两院一体”融合发展建设工作。药事管理水平不断提升,形成具有六院特色的药事质控评价体系和管理模式;多次在上海药事质控督查中排名全市第一,连续多年荣获上海市临床药事管理优秀单位。先后组织5名药师作为医院医疗队成员赴云南省多家医院开展药学帮扶,获医院脱贫攻坚工作突出贡献奖。

临床药学

六院临床药学工作具有深厚的历史底蕴。20世纪80年代初,我院药学前辈张楠森教授等专家在国内率先提出了临床药学的概念,并在六院开设了全国第一批临床药学基地,开始不断地探索与实践个体化给药指导。在国内三级综合性医院中首家建设了静脉用药调配中心。近年来,不断完善临床药师制度体系建设,扩充临床药师队伍,规范化开展抗感染、抗肿瘤、抗凝、镇痛、呼吸系统等各类药物的合理用药服务,推动临床药师全方位融入临床诊疗团队。目前,临床药学服务已实现医院全覆盖,并依据医院学科特点,配备临床药师重点在骨科、ICU、肿瘤内科、普外科、呼吸内科、心内科、妇产科和神经内科等科室开展临床药学服务和管理。2018年六院临床药学获批上海市首批临床药学重点专科,大大推进了一批围绕六院重点学科和服务临床特色的临床药学专科群的建设步伐。

学科特色

六院药学部深入推进药学服务创新实践,开设了以药物咨询和用药指导为主的合理用药咨询门诊及以肿瘤、阿尔茨海默病、呼吸等方向为主的药学门诊。积极探索基于三级-二级-社区医联体的“3+2+1”药学服务体系,形成医联体内全程化管理,被“上海徐汇”、“学习强国”等多家媒体报道,在全国范围内得到积极推广。建立抗感染药物管理服务体系,荣获2023年度“中国医院管理奖·区域优秀奖”。开展精准用药服务,对十余种药品开展治疗药物检测,逐步完善了系统化的个体化用药服务体系。创立“六院药师”科普平台,撰写的科普文章被人民网、健康中国、健康上海12320、新民晚报、澎湃新闻、今日头条和腾讯网等各大媒体传播,累计阅读量超过百万。

教学

六院药学部为上海交通大学医学院博士点,并承担上海交通大学医学院本科的教学任务,是复旦大学药学院、上海健康医学院的实习点,被评为上海健康医学院“示范性实践教学基地”。现有博士生导师3名,硕士生导师3名,累计培养博士、硕士研究生30余人。作为国家卫健委、中国医院协会临床药师培训基地,以及上海市医院协会首批临床药师培训基地,为全国培养了100多名优秀的临床药师。基地连续多年蝉联上海市医院协会“优秀临床药师培训基地”。本着公益理念,每年定期为老年合理用药大学开展各种合理用药培训,解决了众多老年人的用药问题,得到了社会的认可。

学术研究

六院药学部聚焦临床用药问题、推动科研创新的转化研究,取得了一系列的科研成果。曾获上海市科技进步奖三等奖、上海市医学科技奖三等奖、中西结合科学技术奖一等奖等。近年获批包括国家自然科学基金在内的各类课题60余项,发表论文300余篇,其中SCI论文近百篇。申请国家专利17件,授权5件。重视科研成果的转化,多个院内制剂获批上海市药品监督管理局院内制剂批文。

转型人才

六院药学部一直以来重视人才的培养及药师的转型发展。在人才培养方面,1名药师入选国家“香江学者”计划,1名药师获上海市级浦江人才计划,并作为全市首个药师入选上海市卫生系统优青计划,5名药师先后入选上海市优秀青年临床药师培养计划和医苑新星等项目。积极推动药师药学服务的转型工作,在处方审核、处方点评、药学监护等合理用药管理方面彰显出药师价值。青年药师积极参与技能大赛,先后获得了2020年中国药师职业技能大赛全国总决赛冠军、华东赛区一等奖;2021、2022上海市“奋斗杯”职业药师技能大赛一等奖等好成绩。

Department of Pharmacy, Shanghai Sixth People’s Hospital Affiliated to Shanghai Jiao Tong University School of Medicine

Shanghai Sixth People's Hospital, affiliated with Shanghai Jiao Tong University School of Medicine (referred to as the Sixth Hospital), takes pride in hosting one of Shanghai’s pioneering key clinical pharmacy specialties. Our department is a doctoral training base for the Pharmaceutical Sciences program at Shanghai Jiao Tong University School of Medicine and the Traditional Chinese Medicine program at Shanghai University of Traditional Chinese Medicine, featuring a dedicated clinical pharmacy teaching and research section.Our department holds leading roles in several esteemed organizations, serving as the primary and vice-chair unit for various specialized committees, includingthePharmaceutical Services Specialized Committee of the Chinese Pharmaceutical Association, theClinical Drug Evaluation Specialized Committee of the Chinese Pharmacists Association, the Clinical Pharmacy Specialized Branch of the Chinese Medical Association, the Clinical Pharmacy Management Specialized Committee of the Shanghai Hospital Association, the Hospital Pharmacy Specialized Committee of the Shanghai Pharmaceutical Association, and the Clinical Pharmacy Specialized Branch of the Shanghai Medical Association.With a team of 182 pharmacy professionals, including four chief pharmacists and twelve chief pharmacy technicians, fourteen individuals holding Ph.D. or Pharm.D. degrees, we are committed to building a“pyramid-style”discipline system as part of our hospital's“14th Five-Year Plan”and 2035 Vision, with the Department of Pharmacy designated as one of the“Four Major Plateau Disciplines”.

The Department of Pharmacy at Shanghai Sixth People's Hospital comprises more than 20 specialized units, including the outpatient pharmacy, outpatient herbal medicine pharmacy, emergency pharmacy, fever pharmacy, pediatric pharmacy, inpatient pharmacy, Pharmacy Intravenous Admixture Services (PIVAS), Xiaoyangshan pharmacy, drug procurement and policy research department, compounding room, quality control department, clinical pharmacy department, prescription verification center, therapeutic drug monitoring and individualized medication research center, and the clinical pharmacy teaching and research section. Our department plays a pivotal role in the daily operations of the Pharmacy and Therapeutics Committee and is actively involved in the implementation and promotion of policies related to essential drugs, centralized procurement drugs, and national negotiation drugs, ensuring a consistent supply of pharmaceuticals.

Furthermore, we lead and enhance essential pharmacy functions throughout the hospital, encompassing antimicrobial drug management, psychotropic and narcotic drug management, adverse drug reaction monitoring, prescription review, quality control of drug management and pharmacy services, drug pricing negotiations, and drug evaluation and assurance. Additionally, our commitment to translating pharmaceutical achievements into practical applications has yielded significant success in pharmacy management, clinical pharmacy, education, and academic research. The department has been honored with the “2023 Shanghai Women's Civilized Post” award and the 2019 first-class “National Youth Civilization Award” for its outstanding outpatient and emergency pharmacies, in addition to being recognized with the “Model Employee Home of the National Education, Science, Culture, Health, and Sports System” title in 2023.

Pharmaceutical management

The Department of Pharmacy at Shanghai Sixth People's Hospital has made significant strides in integrating the two-hospital system to suit our hospital's unique circumstances. This integration encompasses harmonizing management systems, drug formularies, and quality control standards, effectively executing the “two-hospital integration” development work. Our pharmacy management has continually improved, leading to the establishment of a distinctive quality control evaluation system and management model for the Sixth Hospital. Our endeavors have consistently positioned us at the forefront of Shanghai's pharmacy quality control inspections, and we have been acknowledged as an outstanding unit in clinical pharmacy management in Shanghai for consecutive years. Additionally, we have deployed five pharmacists to provide pharmaceutical support to multiple hospitals in Yunnan Province as part of the hospital’s medical teams, earning the hospital the Outstanding Contribution Award for Poverty Alleviation.

ClinicalPharmacy

The clinical pharmacy at Shanghai Sixth People's Hospital has a rich historical background.In the early 1980s, Professor Zhang Nansen and other pharmacy pioneers at our hospital were at the forefront of introducing the concept of clinical pharmacy in China. They established one of the nation's first clinical pharmacy bases at the Sixth Hospital, initiating continuous exploration and practice in providing individualized medication guidance. Additionally, our hospital was a pioneer among domestic tertiary general hospitals in implementing Pharmacy Intravenous Admixture Services (PIVAS).

In recent years, we have persistently bolstered the clinical pharmacist system, expanded the clinical pharmacist team, and standardized the rational use of various medications, including anti-infectives, antineoplastics, anticoagulants, analgesics, and respiratory drugs. This initiative has propelled the comprehensive integration of clinical pharmacists into clinical treatment teams.

Presently, clinical pharmacy services extend throughout the entire hospital. Dependent on the hospital's disciplinary characteristics, clinical pharmacists are strategically positioned in key departments such as orthopedics,ICU, oncology, general surgery, respiratory medicine, cardiology, obstetrics and gynecology, and neurology to provide specialized clinical pharmacy services and management. In 2018, our clinical pharmacy was accredited as one of the first key clinical pharmacy specialties in Shanghai, significantly advancing the development of a clinical pharmacy specialty group focused on the key disciplines and clinical features of the Sixth Hospital.

Disciplinary characteristics

The Department of Pharmacy at Shanghai Sixth People's Hospital has made substantial progress in advancing the innovative practice of pharmaceutical services. We have established rational medication consultation clinics that focus on drug consultation and medication guidance, as well as specialized pharmacy clinics catering to areas such as oncology, Alzheimer’s disease, and respiratory conditions. We are actively exploring the “3+2+1” pharmaceutical service system based on a tertiary-secondary-community healthcare network, aiming to create comprehensive management within the healthcare network. This initiative has garnered attention from multiple media outlets such as “Shanghai Xuhui” and “Xuexi Qiangguo,” and has received positive visibility nationwide. Our department has also established an anti-infective drug management service system, earning the “China Hospital Management Award · Regional Excellence Award” in 2023. We provide precision medication services, conduct therapeutic drug monitoring for over ten drugs, and are gradually perfecting a systematic individualized medication service system. Furthermore, the “Sixth Hospital Pharmacist” science popularization platform, created by our department, has gained significant attention. Articles written for this platform have been disseminated by major media outlets such as People’s Daily Online, Healthy China, Healthy Shanghai12320, Xinmin Evening News, The Paper, Toutiao, and Tencent, with cumulative readings exceeding one million.

Education

The Department of Pharmacy at Shanghai Sixth People's Hospital serves as a doctoral program site for Shanghai Jiao Tong University School of Medicine. In addition, it fulfills important undergraduate teaching responsibilities for Shanghai Jiao Tong University School of Medicine and provides internship opportunities for Fudan University School of Pharmacy and Shanghai University of Health and Medicine. Notably, it has been recognized as a “Demonstrative Practice Teaching Base” bytheShanghai University of Health and Medicine.

At present, the department has three doctoral supervisors and three master's supervisors, and it has successfully trained over 30 doctoral and master's students. It is also designated as a training base for clinical pharmacists by the National Health Commission and the Chinese Hospital Association. The department has contributed to the training of over 100 clinical pharmacists nationwide and has been consistently honored as an “Outstanding Clinical Pharmacist Training Base” by the Shanghai Hospital Association for several consecutive years.Furthermore, in alignment with its commitment to public welfare, the department conducts regular rational medication training for the elderly at the University of Rational Medication for Seniors each year. This initiative addresses numerous medication-related challenges faced by the elderly and has earned notable social recognition.

Academic research

The Pharmacy Department at Shanghai Sixth People's Hospital is committed to addressing clinical medication concerns and advancing translational research in scientific innovations, resulting in numerous notable research accomplishments. Our department has been honored with awards such as the Third Prize of the Shanghai Science and Technology Progress Award, the Third Prize of the Shanghai Medical Science and Technology Award, and the First Prize of the Chinese and Western Medicine Integration Science and Technology Award, among others.

In recent years, we have successfully secured over 60 research projects, including grants from the National Natural Science Foundation of China, and have published more than 300 papers, almost 100 of which are indexed in SCI. Furthermore, we have filed applications for 17 national patents, five of which have been granted. We prioritize translating research findings into practical applications, and have received approval from the Shanghai Municipal Drug Administration for several in-hospital formulations.

Talent Transformation

The Department of Pharmacy at Shanghai Sixth People's Hospital places significant emphasis on fostering and advancing pharmacy talent. In the realm of talent development, one pharmacist was honored with the national “Hong Kong Scholars Program”, while another pharmacist obtained recognition through the Shanghai Pujiang Talent Program, marking the first pharmacist to be included in the Shanghai Health System Outstanding Young Talent Program. Furthermore, five pharmacists have been chosen to participate in the Shanghai Outstanding Young Clinical Pharmacist Training Program and other notable initiatives, including the Medical Star Program.

We actively promote the transformation of pharmacy services, highlighting the value of pharmacists in rational medication management, including prescription review, prescription evaluation, and pharmaceutical care. Our young pharmacists have actively participated in skill competitions, achieving impressive results such as the national championship in the 2020 China Pharmacist Professional Skills Competition, first prize in the East China Regional Competition, and first prize in the 2021 and 2022 Shanghai“FendouCup”Professional Pharmacist Skills Competition.
