上海交通大学医学院附属胸科医院药学部

上海市胸科医院（暨上海交通大学医学院附属胸科医院），创建于1957年，为我国最早建立的集医疗、教学、科研为一体的，以诊治心、肺、食管、气管、纵隔疾病为主的三级甲等专科医院，拥有国家重点学科1个，国家临床重点专科3个，国家中医药管理局“十二五”重点专科1个。

上海市胸科医院药剂科下设门急诊药房、中药房、药库、住院药房、静脉药物配置中心、临床药学室六个部门，承担全院药品质量管理、临床药学实践、药学科研等药学相关工作。其中临床药学室成立于2007年，现有专职临床药师11人，包括副主任药师2名，主管药师3名，其中博士学历5人，硕士学历6人。

药房是提供药学服务的一大窗口，与时俱进引进自动化智能发药设备，并设立咨询窗口，配备专业的药学技术人员为患者提供二十四小时全天候服务。

静脉药物配置中心（PIVAS）建成于2003年6月，建筑面积300平方米，包括抗生素及细胞毒性药物配置间、普通及肠外营养药物配置间，配备3台A/B3型生物安全柜及2台特殊设计的水平层流台，可同时供10多名专业技术人员在局部百级的环境下进行无菌配置。为满足医院需要，将在日后扩展静脉配置中心规模。依靠先进的净化设备和严格的科学管理，在受过良好培训的专业技术人员的严谨工作下，配置中心为病人提供高质量、安全、有效的静脉输注药物。

药剂科自2000年开展药师上临床工作，选择ICU病房为重点，由两名临床药师长期驻扎ICU病房，与医护人员共同工作。随着临床药师队伍的不断扩大，越来越多年轻药师取得临床药师资质，目前各临床科室已基本实现临床药师全覆盖。临床药师利用自身的专业知识向医师提供药物方面的信息和服务：介绍新药、为特殊病人设计个体化给药方案、及时满足病人对药品的特殊需求；并通过共同查房、进行单独的药学查房，探视病人、阅读病史、撰写药历。运用临床药学专业知识对用药进行审核，为临床“安全、合理、经济”用药提供专业保证。

开展临床药师查房、抗感染治疗会诊、特殊使用类抗菌药物审核、临时进药审核、药物信息咨询、患者用药教育等临床药学工作，每季度出版药讯，每月进行抗菌药物、门急诊、住院处方点评，提升药学服务质量。持续开展合理用药专项处方点评工作，为医务部门用药监控管理提供数据支撑；强化特殊药品管理，建立门诊癌痛患者麻醉专用病历，参与规范化癌痛治疗病房的创建工作；建立精二处方电子专册登记，确保用药安全。药品不良反应监测工作获得徐汇区先进。

临床药学室近年来着重发展与临床相结合的科研工作。2019年至今已获得国家自然科学基金课题3项，其中青年项目2项，面上1项。此外，教学方面承担了交通大学医学院、交通大学药学院博士和硕士研究生的培养工作，于2022年10月取得交通大学药学院教育教学实践基地，承担本科生校外见习工作。

Shanghai Chest Hospital (also known as Shanghai Chest Hospital, Shanghai Jiao Tong University School Of Medicine), established in 1957, is one of the earliest hospitals in China that integrates medical treatment, teaching, and research, specializing in the diagnosis and treatment of heart, lung, esophageal, tracheal, and mediastinal diseases. The hospital has one national key discipline, three national clinical key specialties, and one key specialty supported by the National Administration of Traditional Chinese Medicine during the "Twelfth Five-Year Plan".

The Pharmacy Department of Shanghai Chest Hospital is divided into six sections: outpatient pharmacy, traditional Chinese medicine pharmacy, pharmacy warehouse, inpatient pharmacy, pharmacy intravenous admixture services, and clinical pharmacy department. It is responsible for the quality management of all hospital medicines, clinical pharmacy practice, and pharmaceutical research and other pharmacy-related work. The clinical pharmacy department was established in 2007 and currently has eleven full-time clinical pharmacists, including two associate chief pharmacists and three pharmacists-in-charge, with five pharmacists holding a doctor’s degree and six holding a master's degree.

The pharmacy is a major window providing pharmaceutical services, keeping pace with the times by introducing automated and intelligent drug dispensing equipment, and setting up a consultation window with professional pharmaceutical technical personnel to provide 24-hour service for patients.

The Pharmacy Intravenous Admixture Services (PIVAS) was established in June 2003, covering an area of 300 square meters, including rooms for the preparation of antibiotics and cytotoxic drugs, as well as rooms for ordinary and parenteral nutrition drugs. It is equipped with three type A/B3 biosafety cabinets and two specially designed horizontal laminar flow tables, allowing more than 10 professional technical personnel to perform sterile preparation in a local class 100 environment. With advanced purification equipment and strict scientific management, under the rigorous work of well-trained professional technical personnel, the preparation center provides high-quality, safe, and effective intravenous infusion drugs for patients.

Since 2000, the pharmacy department has carried out the transition of pharmacists going to the clinic, selecting the ICU ward as the focus, with two clinical pharmacists stationed in the ICU ward for a long time, working together with the medical staff. As the clinical pharmacy team continues to expand, more and more young pharmacists have obtained clinical pharmacy qualifications, and currently, clinical pharmacists have basically achieved full coverage in all clinical departments. Clinical pharmacists use their professional knowledge to provide information and services on drugs to doctors: introducing new drugs, designing individualized drug plans for special patients, and meeting patients' special drug needs in a timely manner; and through joint rounds, conducting separate pharmaceutical rounds, visiting patients, reading medical records, and writing medical records. Clinical pharmacists use their professional knowledge to review medication, providing professional assurance for "safe, rational, and economical" clinical medication use.

Clinical pharmacy services such as clinical pharmacist rounds, anti-infection treatment consultations, review of special use class antibiotics, temporary drug review, drug information consultation, and patient medication education are carried out. A pharmaceutical newsletter is published every quarter, and monthly reviews of antibiotics, outpatient and emergency prescriptions, and inpatient prescriptions are conducted to improve the quality of pharmaceutical services. The special prescription review work for rational drug use is carried out continuously to provide data support for the monitoring and management of drug use by the medical department; special drug management is strengthened, a special medical record for outpatient cancer pain patients is established, and the creation of standardized cancer pain treatment wards is participated in; an electronic special register for psychotropic prescription is established to ensure drug safety. The adverse drug reaction monitoring work has been advanced in Xuhui District.

In recent years, the clinical pharmacy department has focused on developing scientific research work combined with clinical practice. Since 2019, it has obtained three projects from the National Natural Science Foundation of China. In addition, in terms of teaching, it has undertaken the training of doctoral and master's students from the School of Medicine and the School of Pharmacy of Shanghai Jiao Tong University, and in October 2022, it became a teaching and education practice base of the School of Pharmacy of Shanghai Jiao Tong University, undertaking the off-campus internship work for undergraduate students.
