上海交通大学医学院附属精神卫生中心(上海市精神卫生中心)药学部

上海市精神卫生中心是精神专科医院,从建院以来,医院就发挥着药品供应保障、药事管理等职能,在医药改革形势的激励下,在院领导的支持下和药学人员积极努力下,主动学习提高技能,促进药学服务转型。药学部(总院)共有32人,主任药师1名,副主任药师1名,主管药师19名;其中研究生7人,本科17名,本科学历达到87.5%;有7名专职临床药师。药学部主任毛叶萌,主任药师,是我中心精神科临床药学特色学科带头人,也是上海市医学会临床药学专科分会精神药物学组组长。为了促进精神科临床药学学科建设,推动全国精神科临床药师技能的提高,作为项目负责人,从2015年起开展全国继续教育培训班-精神科药学监护新进展培训班,目前已开展8届,培训了来自全国各大精神专科医院的临床药师900余人次,得到学员们的好评。2022年药学部申请并获批第十四批中国医院协会临床药师培训基地。

药学部在分管领导直接领导下,负责药事管理工作,药学部下设:药库、调剂部门(门急诊药房、住院药房)、临床药学室、ADR监测办公室,负责医院的药品保障、药事管理和药学服务、ADR监测及上报工作等。药学部从2007年开始设置临床药学室,至今已经17年有余。临床药学室开展以合理用药为核心的临床药学工作,临床药师参与临床疾病诊断、治疗,提供药学技术服务,包括药物重整、药学监护、用药教育、药学门诊。同时与医疗、检验合作开展了药物基因检测和TDM等相关工作,建立个体化药学服务平台,为患者个体化用药方案的制订服务。 2016年2月至2018年7月开展了21个品种药品、17个药物基因位点进行基因检测,合计约1500例患者,临床药师根据患者病情及躯体情况提出个体化用药指导报告。

依托医院精神科临床药学特色学科建设,建立精神科的合理用药数据库;建立从患者入院起到出院随访的符合精神科特色的全程化药学监护体系;

药学部临床药学室还将在临床工作中发现的问题、精神科相关药物治疗国内外进展和信息、医院处方/医嘱点评内容、病例分析、药品不良反应和药学相关的政策法规等内容,编写《精中药讯》1份/季度,为临床合理用药提供参考。并根据本中心新进药品情况,编写上海市精神卫生中心处方集。临床药学同时参与编写了《精准医学:药物治疗纲要》和翻译Medication Therapy Management相关精神科章节,以及药物科普丛书。

药学部非常重视药物不良反应的监测工作,在院内做ADR的宣教,并联合医务、护理共同做好ADR的监测及上报工作 ,临床药师在药学服务过程中发现的ADR均主动收集,并统一上报属地药监局。2011年-2016年、2018-2023年连续获得徐汇区药品不良反应监测工作优秀集体奖,以及毛叶萌、马燕、李方捷和李红分别获得2011-2023年度徐汇区药品不良反应监测工作优秀个人奖10余次。

药学部多次被评为上海市质控优秀单位,2012年和2014年获得上海市临床药师管理优秀单位称号。


Shanghai Mental Health Center

The Shanghai Mental Health Center (SMHC) is a psychiatric specialty hospital. Since its establishment, the hospital has been fulfilling its functions in drug supply assurance and pharmaceutical management. Motivated by the reform of the pharmaceutical sector, with the support of the hospital leadership and the active efforts of the pharmacy staff, the hospital has taken the initiative to learn and improve skills to promote the transformation of pharmaceutical services. The Department of Pharmacy (Xuhui Branch) has a total of 32 staff members, including 1 Chief Pharmacist, 1 Deputy Chief Pharmacist, and 21 Senior Pharmacists; among them, there are 7 with graduate degree and 17 with a bachelor's degree, with the proportion of undergraduate education reaching 87.5%; there are 7 full-time clinical pharmacists. The Director of the Pharmacy Department, Mao Yemeng, Chief Pharmacist, is the leading figure in the specialty of clinical pharmacy of SMHC's psychiatry and also the head of the Psychiatric Pharmacy Group of the Clinical Pharmacy Specialty Branch of the Shanghai Medical Association. To promote the construction of the discipline of clinical pharmacy in psychiatry and to improve the skills of clinical pharmacists in the field of psychiatry nationwide, we has been conducting the National Continuing Education Training Class - New Advances in Psychiatric Pharmacy Monitoring as the project leader since 2015, which has been held for 8 sessions, training more than 900 clinical pharmacists from psychiatric hospitals across the country, and has received high praise. In 2022, the Pharmacy Department applied for and was approved as the 14th batch of the Clinical Pharmacy Training Base by the Chinese Hospital Association.

The Pharmacy Department, under the direct leadership of the responsible leaders, is in charge of pharmaceutical affairs management. We includes the following sections: the pharmacy warehouse, the dispensing department (outpatient pharmacy, inpatient pharmacy), the clinical pharmacy office, and the Adverse Drug Reaction (ADR) monitoring office. We responsible for the hospital's drug supply assurance, pharmaceutical affairs management, pharmaceutical services, ADR monitoring, and reporting, among others. The Clinical Pharmacy Office was established in 2007 and has been in operation for over 17 years. The office carries out clinical pharmacy work centered on rational drug use, with clinical pharmacists participating in the diagnosis and treatment of clinical diseases, providing pharmaceutical technical services, including pharmaceutical reconciliation, pharmaceutical monitoring, pharmaceutical education, and pharmaceutical clinic services. In collaboration with medical and laboratory services, we have also initiated pharmacogenetic testing and therapeutic drug monitoring (TDM), establishing a personalized pharmaceutical service platform to serve the formulation of individualized medication plans for patients. During February 2016 and July 2018, approximately 1,500 pharmacogenetic testings were conducted on 21 drugs and 17 drug-related gene loci. Clinical pharmacists have provided personalized medication guidance reports based on the patients' symptoms and physical status.

Base on the construction of the hospital's psychiatric clinical pharmacy specialty, a rational drug use database for psychiatry has been established; a comprehensive pharmaceutical care system that is in line with the characteristics of psychiatry, from patient admission to discharge follow-up, has been established.

The Clinical Pharmacy Office also compiles the "Psychopharmaceutical Newsletter" quarterly, which includes issues identified in clinical work, domestic and international advancements and information related to psychiatric drug therapy, hospital prescription/order review content, case analysis, adverse drug reactions, and pharmaceutical-related policies and regulations, to provide a reference for rational drug use in clinical practice. Additionally, based on the new drugs introduced by the center, compiles the Formulary of SMHC. The Clinical Pharmacy Office has also participated in the compilation of "Precision Medicine: Drug Therapy Guidelines" and the translation of the psychiatric chapters related to“Medication Therapy Management”, as well as a series of popular science books on drugs.

The Pharmacy Department places great emphasis on the monitoring of adverse drug reactions (ADRs), conducting ADR awareness within the hospital, and working in conjunction with medical and nursing departments to monitor and report ADRs. All ADRs identified by clinical pharmacists in the process of pharmaceutical services are actively collected and uniformly reported to the local drug regulatory authority. From 2011 to 2016 and 2018 to 2023, our department has consistently received the Xuhui District Award for Excellence in ADR Monitoring, and individuals such as Mao Yemeng, Ma Yan, Li Fangjie, and Li Hong have received the Xuhui District Award for Excellence in ADR Monitoring for individuals more than 10 times from 2011 to 2023.

The Pharmacy Department has been repeatedly rated as an excellent unit for quality control in Shanghai and has been awarded the title of Excellent Unit for Clinical Pharmacy Management in Shanghai in 2012 and 2014.
