上海交通大学医学院附属上海儿童医学中心临床药师培训基地
国家儿童医学中心(上海)、上海交通大学医学院附属上海儿童医学中心是一所集医、教、研于一体的三级甲等儿童专科医院,拥有心血管、血液肿瘤和发育行为儿科三个国家重点学科以及儿科重症、小儿外科、儿童肿瘤、儿童影像、儿童感染、儿童营养等六个国家临床重点专科。作为国内最早开展临床药学工作的单位之一,上海儿童医学中心已建立了一套规范化的临床药师工作体系。专科临床药师深度参与疾病的诊断、治疗和用药方案的制定和评估,极大地促进了临床合理用药工作的开展。2019年10月被批准为第十一批国家卫健委、中国医院协会临床药师培训基地,2021年入选国家卫健委紧缺人才临床药师培训项目,2024年荣获国家卫健委首批“驻科药师”试点单位。
药剂科主任李志玲主任药师担任培训基地负责人,现有临床药师20名,其中5名临床药师带教师资,涵盖了多个儿科专业,包括血液肿瘤、肠外肠内营养、抗感染、新生儿、心血管、呼吸等专业方向。
培训围绕患者临床诊治展开学习,以扎实的理论学习为基础,通过深入学习药事法规、系统学习专业基础知识、积极参与各类学术讲座,结合个人的思考和实践总结,不断提升自身的专业水平;以临床实践为核心,开展医学、药学查房,深入进行重点病例的分析与讨论,针对危重患者参与多学科团队会诊,并为患者提供用药教育及长期随访管理;在次基础上,我们坚持医药结合为原则,将临床药物治疗作为重点,以患者疾病为脉络,围绕具体的药学问题,实现个体化用药。我们采用“实践为主,教学相长”的培训模式,旨在通过不断的实践和教学互动,提升临床药师的专业技能。目前已培养学员16名,他们通过系统的专业培训,具备了扎实的理论知识和丰富的临床药学实践经验。
Clinical Pharmacists Training Base,Shanghai Children's Medical Center affiliated to Shanghai Jiao Tong University School of Medcine
National Children's Medical Center (Shanghai),Shanghai Children's Medical Center (SCMC) affiliated to Shanghai Jiao Tong University School of Medcine, is a Grade A tertiary hospital specialized in children, integrating medical treatment, education, and research. The hospital boasts three national key disciplines in cardiology, hematology-oncology, and developmental-behavioral pediatrics, as well as six national clinical key specialties, including pediatric intensive care, pediatric surgery, pediatric oncology, pediatric imaging, pediatric infectious diseases, and pediatric nutrition. As one of the earliest institutions in China to engage in clinical pharmacy, Shanghai Children's Medical Center has established a standardized working system for clinical pharmacists. The pharmacists play a pivotal role in disease diagnosis, treatment, and medication regimen formulation and evaluation, significantly promoting rational drug use inclinical settings. In October 2019, the center was approved as one of the 11th batch of clinical pharmacists training bases by the National Health Commission of the People's Republic of China and the Chinese Hospital Association. In 2021, it was selected for the National Health Commission's clinical pharmacists training project for scarce talents. And in 2024, it was honored as one of the first pilot sites for the "Resident Pharmacist" program by the National Health Commission.
Dr. Li Zhiling, the director of the pharmacy department, serves as the head of the training base, which currently has 20 clinical pharmacists, including 5 teaching mentors. These pharmacists cover various pediatric specialties, such as hematology-oncology, parenteral and enteral nutrition, anti-infection, neonatology, cardiology, and respiratory medicine.
The training revolves around patients' clinical diagnosis and treatment, emphasizing solid theoretical foundations through in-depth studies of pharmaceutical regulations and professional basics. Participants actively engage in various academic lectures, reflecting on their practices and summarizing experiences to continuously enhance their professional skills. The training emphasizes clinical practice, where medical and pharmaceutical ward rounds are conducted, and in-depth case analyses and discussions are held. Trainees participate in multi-disciplinary team consultations for critically ill patients and provide medication education and long-term follow-up management for patients. With an adherence to the integration of medicine and pharmacy, the program focuses on clinical drug therapy, tailored to patients' specific medical conditions and pharmaceutical issues, aiming for individualized medication. Adopting a "practice-oriented, mutually beneficial teaching and learning" approach, the training aims to enhance clinical pharmacists' professional skills through continuous practice and teaching interaction. So far, 16 trainees have been cultivated, equipped with solid theoretical knowledge and rich practical experience in clinical pharmacy.
