耳鼻咽喉头颈外科简介

上海交通大学附属第六人民医院耳鼻咽喉-头颈外科,2011年首批入选国家卫计委重点临床专科,2012年入选上海市卫计委“重中之重”临床重点学科,也是上海交通大学医学院的首批重点学科,为上海交通大学医学院首批耳鼻咽喉科学博士、硕士学位研究生培养点、博士后流动站,以及苏州大学硕士学位研究生培养点。设有上海市听力测试中心、上海交通大学眩晕疾病诊治中心,上海市第六人民医院鼾症诊治中心,以及过敏原检测、免疫治疗中心,牵头成立上海市第六人民医院头颈肿瘤治疗协作组、上海东方耳鼻咽喉科研究所、上海交通大学耳鼻咽喉科研究所。

学科分设耳科、鼻科和咽喉头颈外科三个亚学科,在耳显微外科、耳神经外科、侧颅底外科、鼻颅底外科、鼾症外科、头颈外科等领域具有显著优势。在中国医学科学院“医院科技影响力”专科科技影响力排行榜、北京大学“中国最佳临床学科评估”排行榜、复旦大学“中国医院专科声誉排行榜”等国内主流榜单上全国排名713位。

学科人力资源充实,至2019年医师队伍九成以上为研究生学历,六成以上具有博士学位,高级职称15名,硕士研究生导师7名,博士研究生导师5名。学科大力开展科学研究与医学教育,牵头成立的上海市睡眠呼吸疾病重点实验室、上海交通大学耳鼻咽喉科研究所成为市内外科学研究、医学教育及人才培养的重要基地。开展近50期国家级医学继续教育项目、6期上海市耳鼻咽喉头颈外科研究生暑期学校,为我国培养专科人才数千名。

5年来学科承担国家级、省部级课题40余项,获各项基金资助4000余万元,先后发表重要学术论文370余篇,其中SCI期刊收录论文180余篇,学科排名前30%5分以上文章数十篇;获得省部级科技进步奖8项,获奖总数位列国内同级同类学科前列;学科有多人入选国家自然基金杰出青年基金、国家人社部百千万人才工程、上海市十佳医生、上海市领军人才、上海市优秀学科带头人、上海市科委启明星计划、上海市卫健委百人计划、上海市教委双百人计划等高层次人才培养项目

临床服务能力介绍

掌握先进的耳显微、耳神经外科及侧颅底外科技术。在国内率先开展耳道上径路以及多种疑难、复杂病例的耳蜗植入,是国内常规、规模化开展听神经瘤、颈静脉孔区肿瘤等侧颅底外科手术的少数中心之一。掌握先进的听力重建技术,率先开展一次成形二氧化碳激光辅助人工镫骨手术。开设耳鸣专科门诊,提供声音治疗、习服治疗、重复经颅磁刺激治疗(rTMS)等多种治疗方案。开设眩晕专科门诊,依托上海市卫计委新技术推广项目,协助10家三级甲等医院、10家社区卫生服务中心建立BPPV诊治网络。掌握梅尼埃病阶梯治疗的全部技术,为患者提供个性化精准治疗。开展听觉病理生理、感音神经性聋发病机制及拮抗措施的系列研究。

掌握先进的鼻内镜外科、鼻眼相关外科、功能性鼻整形外科、鼻内镜颅底外科技术。在国内率先报道经鼻内镜蝶腭动脉电凝切断治疗顽固性鼻出血以及带血管蒂的鼻中隔黏骨膜瓣修复鼻颅底缺损。率先开展经蝶窦、经蝶骨平台鞍结节入路侵入第三脑室巨大垂体瘤的内镜下切除。开展低温等离子辅助鼻恶性肿瘤鼻内镜下切除及一期颅底重建。是国内常规、规模化开展垂体瘤等前颅底肿瘤内镜下手术切除的少数中心之一。

掌握先进的咽喉头颈外科诊治技术,尤其擅长OSAHS的外科治疗。研发便捷、高效的中国人群OSAHS筛查模型;建立基于阻塞平面定位的症外科手术策略,开展双颌前徙术(MMA)治疗重度OSAHS,探索悬雍垂腭咽成形术(UPPP)等外科技术治疗重度OSAHS的疗效及其影响因素并建立OSAHS外科治疗精准适应症,大幅度提高手术疗效;从OSAHS分子遗传学、目标脏器损伤及其机制、OSAHS诊治新技术三个主要方向,开展系列OSAHS随机对照、队列、病例对照、本横断面临床及基础研究,探索国人OSAHS遗传背景,OSAHS代谢紊乱特征及其机制。掌握先进的涎腺良恶性肿瘤、口腔良恶性肿瘤、甲状腺良恶性肿瘤、咽喉良恶性肿瘤的规范化、个体化治疗技术,掌握中晚期头颈肿瘤切除及功能重建技术,开展颌面骨折坚强内固定技术。

上海东方耳鼻咽喉科研究所介绍

上海东方耳鼻咽喉科研究所成立于2010年,在殷善开教授的带领下,促进耳鼻咽喉科的基础研究和临床应用研究,加快新技术的开发和临床应用,造福病人。研究所长期致力于耳聋、耳鸣、耳源性眩晕、鼾症、头颈肿瘤、鼻变态反应等病理生理机制研究,2019年单体面积全市最大的“科研综合楼”投入使用,研究所专用面积1500平米,极大地提升耳鼻咽喉研究所的科研工作条件。设有电生理实验室、膜片钳实验室、分子生物学实验室、病理生理实验室、低氧病理实验室、斑马鱼模式生物实验室和颞骨解剖实验室。拥有独立的研究设施及精良的研究设备,包括Western blot系统和ChemiDoc XRS化学发光系统,MyiQ单色实时PCR检测系统,Nikon 80i明视场/荧光显微镜和图像系统,Leica冰冻切片机,ZEISS双光子与荧光显微共聚焦,Nikon/倒置相差显微镜,解剖显微镜等设备,全方位的满足科研所需。

上海市睡眠呼吸障碍疾病重点实验室介绍

20181211日,上海市科委发布“关于2018年度上海市重点实验室建设立项的通知”。经过遴选,殷善开教授领衔申报的“上海市睡眠呼吸障碍疾病重点实验室”成功获批。上海市睡眠呼吸障碍疾病重点实验室瞄准以阻塞性睡眠呼吸暂停低通气综合征(OSAHS)为代表的睡眠呼吸障碍疾病的国际前沿领域,针对我国睡眠呼吸障碍疾病的重大问题,结合国家和上海市中长期科技发展战略和重大民生工程,高效整合上海市第六人民医院和市内外、国内外相关单位在睡眠呼吸障碍疾病临床和基础领域的优质资源,搭建更加坚实高效的研究平台;以OSAHS为主要研究方向,兼顾其他睡眠呼吸障碍疾病,在病因学、遗传与环境的交互作用、疾病发生发展机制、生物标志物、诊断与治疗新技术研发及临床转化应用等关键领域取得标志性原创成果;以睡眠呼吸障碍疾病诊治及研究的高端技术支撑体系和创新平台为支撑,利用物联网技术及我国庞大病患群体的大数据优势,建立适合我国国情的、原创性的预测、预防、个体化和参与性的睡眠呼吸障碍疾病现代医学模式和诊疗策略,并建立一支在睡眠呼吸障碍疾病领域具有全球一流水准和一定影响力的科研临床队伍

实验室现有固定人员 57 人,其中教授 13名,国家人社部百千万人才工程 1名,国家杰出青年基金获得者3名,教育部长江学者1名,973首席科学家2名,上海市领军人才2名,上海市优秀学科带头人1名,上海市卫健委百人计划2名,上海市青年科技启明星及跟踪计划各 1名。

代表性科研成果:

1OSAHS生物样本库及专病数据库的建立

建立了目前全球单中心样本量最大的OSAHS生物样本库及专病数据库,基于此样本库发表了重要学术论文30余篇。

 

2OSAHS遗传风险位点的鉴定

完成了超过两万例OSAHS患者及健康对照人群的GWAS分析,首次在全球范围内发现与OSAHS发病相关的全局阳性SNP位点。

 

3OSAHS临床新技术研发与推广应用

构建基于云平台的OSAHS自动诊断系统,开发便携睡眠监测仪;提出OSAHS个体化序列治疗策略及开展精准外科治疗,取得显著疗效。

At the initial establishment of Shanghai Sixth People's Hospital, otorhinolaryngologydepartment was established. In the long history of more than one hundredyears, the department has witnessed the historical development course ofthe Otorhinolaryngology Head and Neck Surgery in our country, making greatprogress at the same time. As a national key clinical specialty and a keydiscipline with top priorityin Shanghai, the discipline is divided into three subdisciplines:otology, nose as well as pharynx larynx head and neck surgery. Everymedical staff of Otorhinolaryngology Head and Neck Surgery adheres to thehospital spirit of Benevolence, Integrity, Dedication, Innovation, continuouslyimproves professional knowledge and skills, enhances service awarenessand level, and provides better medical treatment and services to patients withadvanced concepts and first-class technology. At the same time, we vigorouslycarry out scientific research and medical education. Shanghai Key Laboratoryof Sleep Breathing Disorders and Otorhinolaryngology Institute, ShanghaiJiaotong University have become important bases for scientific research, medicaleducation and personnel training in and outside the city.The department ranked the seventh in the Best Department ReputationRankingby Fudan University Hospital Management Research Institute in2017; ranked 10th in the country in the Chinas Best Clinical DisciplineAssessmentby Peking University, and ranked 13th in the country in specialisttechnology influence ranking of Hospital Technology Influenceby ChineseAcademy of Medical Sciences.

We master advanced ear microsurgery, ear neurosurgery and lateral skullbasesurgery. As one of the few centers in China to carry out the routineand large-scale operation of lateral skull base surgery such as acousticneuroma and jugular foramen tumor, we took the lead in China to carry outcochlear implantation with suprameatal approach for a variety of difficult andcomplicated cases. Meanwhile, we master advanced hearing reconstructiontechnology and initiatively perform once-forming carbon dioxide laser assistedartificial stapes surgery. We set up a special clinic for tinnitus, providing avariety of treatments including voice therapy, retraining therapy, and repetitivetranscranial magnetic stimulation(rTMS). There is also a vertigo specialist clinic,which assists 10 tertiary hospitals and 10 community health service centers inestablishing a BPPV diagnosis and treatment network by relying on ShanghaiMunicipal Health and Family Planning Commission's new technology promotionproject. We master all the techniques of Meniere's disease escalation therapyand provide personalized and precise treatment for patients. Also, we conduct a series of studies on pathogenesis and antagonistic measures forpathophysiology of hearing and sensorineural deafness.We are master in endoscopic sinus surgery, functional rhinoplasty, nose andeye related surgery, as well as endoscopic skull base surgery. We initiatively reported endoscopic sphenopalatine electrocoagulation for treatment ofrefractory epistaxis and skull base repair with nasal septum mucosa pedicleflap interiorly. We were also the first to perform endoscopicresection of giantpituitary tumor with invasion of third ventricle via trans-sphenoidal sinus,planumsphenoidale and tuberculumsellae surgical approaches. We have further developed low-temperature plasma-assisted endoscopic resection ofnasal malignant tumor and skull base reconstruction in one-stage. We are oneof the few centers in China routinely performed endoscopic surgical resectionfor tumors in anterior skull base such as pituitary tumors.

We master advanced laryngeal, head and neck surgery diagnosis andtreatment techniques, especially expert on surgical treatment of OSAHS.We developed a convenient and efficient OSAHS screening model forChinese population; established a surgical strategy for snoring based on different localization of block plane, perform maxillomandibularadvancement (MMA) for severe OSAHS patients, explore treatment effectof uvulopalatopharyngoplasty(UPPP) surgery for severe OSAHS patientsand all those factors that can affect treantment effect, and establish accurateindications for OSAHS surgical treatment, improve surgical efficacy forOSAHS;To explore Chinese OSAHS patients’ genetic background ,characteristics and mechanisms of OSAHS metabolic disorder, we conductseries of OSAHS randomized control, cohort, case-control, large-samplecross-sectional clinical and basic researches. Those researches mainly focused on three directions which include molecular genetics, target organdamage and its possible mechanism, new technologies for OSAHS diagnosis.

Shanghai Oriental Institute of Otorhinolaryngology was established in 2010.Under the leadership of Professor YinShankai, it promotes basic research and clinical application research inotorhinolaryngology, accelerates the development and clinical application ofnew technologies for the benefit of patients.The institute director, who has long been committed to the study ofpathophysiological mechanisms of deafness, tinnitus, otogenic vertigo,snoring, head and neck cancer, nasal allergy, has independent researchfacilities and sophisticated research equipment, including electrophysiologicallaboratory, patch clamp experiment laboratory, molecular biology laboratory,pathophysiology laboratory, hypoxic pathology laboratory, zebrafish modelbiology laboratory. The largest “scientific research building” in the citywhich is a key construction project of Shanghai “Twelfth Five-Year Plan”was put into use. The research institute has a specific area of 1,500square meters, which greatly enhanced research work conditions forInstitute of Otorhinolaryngology.

On December 11, 2018, the Shanghai Municipal Science and TechnologyCommission issued the “Notice on the Establishment of Shanghai KeyLaboratory Construction in 2018”, and “Shanghai Key Laboratory of SleepDisordered Breathing” was successfully approved. This marks another majorbreakthrough of our hospital following the establishment of Shanghai KeyLaboratory of Diabetes Mellitus.After years of effort, this key laboratory has formed an energetic team consisting ofchief physicians/professors, deputy chief physicians/associate professors, talentedyoung physicians/teachers and visiting professors. Based on the Department ofOtorhinolaryngology Head and Neck Surgery of Shanghai Jiao Tong UniversityAffiliated Sixth People’s Hospital, the laboratory organically integrates theclinical and research resources of Shanghai Jiaotong University School ofMedicine (School of Basic Medical sciences, School of Public Health), ShanghaiMedical College of Fudan University (School of Basic Medical sciences, Schoolof pharmacy), forming an innovative research team with the interdisciplinarity cooperation between clinical medicine with basic medicine, preventive medicine,environmental medicine, pharmacy, materials science and etc.. The laboratoryhas 1 person in the Million Talents Project of Ministry of Human Resources andSocial Security, 3 persons won the National Science Fund for DistinguishedYoung Scholars, 1 person as Yangtze River Scholar, 2 persons as 973 ProgramChief Scientist, 2 persons as Shanghai Leading Talents, 1 person as ShanghaiExcellent Academic Leaders, 2 persons in Hundred Talents Program of ShanghaiHealth and Family Planning Commission, 1 person in Academic Leader andNew Hundred Talents Program of Shanghai Jiao Tong University School ofMedicine, 1 person in Science and Technology Innovation Action Plan andTracking Program. For the key laboratory members, 32 were members of nationalsocial organizations, 25 were members of social organizations of Shanghai.
