上海交通大学医学院附属松江医院药剂科
上海交通大学医学院附属松江医院药剂科作为集药品供给与管理、临床用药的指导与监督、社区临床药师的教学与培养为一体的综合性技术部门，建立以病人用药安全为中心的管理模式，开展以合理用药为核心的临床药学工作。下设临床药学室、门急诊西药房、中药房、感染科药房、发热药房、住院药房、手术室药房、静脉药物配置中心、药库9个部门。药剂科现有职工70人，其中主任药师3人，副主任药师2人，主管药师45人。其中博士1人，硕士9人，硕士生导师2人。2019年获评松江区医学重点学科A类建设项目，院级重点学科B类，2021年正式成为上海市临床药学重点专科。
近年来，药剂科不断推进医院药学信息化建设，先后引入自动发药机、智能麻醉柜（语速调慢）、智能物流机器人、住院药品自动分拣机、全自动静脉配药机器人等先进设备，逐步形成现代化的药品管理模式。在保证药品供应的基础上，药剂科积极开展药学服务项目，目前有专职临床药师14人，实行临床药师病区责任制，以参与临床查房、病例讨论、用药宣教等多种形式参与临床诊疗。2018年，原松江区中心医院药剂科主任陈坚领衔的“合理用药创新工作室”被上海市总工会授牌命名“劳模创新工作室”，积极主动将服务前移，满足了门诊患者对合理用药知识的需求。2019年12月开设药学门诊，依托基因检测技术开展华法林、氯吡格雷及他汀类药物基因检测，为患者提供个体化用药服务，2022年推进药学门诊专科化建设，现有精准药学门诊、抗凝药学门诊、抗感染药学门诊、抗肿瘤药学门诊、妇儿药学门诊等多个亚专科药学门诊。2019年我院正式通过药物临床试验基地验收并于药剂科成立药物临床试验机构办公室，配备有专用场地与足够的设施设备，目前已完成呼吸内科、心血管内科、急诊医学科3个专业组备案，完成3个药物临床试验项目，另有3个项目处于立项阶段。
作为区域医疗中心，药剂科积极提升药学辐射能力，推动医联体内药事服务质量及药学服务能力共同提升，逐渐形成区域一体化药学服务中心。2016年起药剂科与18家社区卫生服务中心形成结对帮扶机制，促进药事管理与药学服务共同提升。2020年药剂科作为主要牵头单位推进松江区域审方中心项目建设，实现区域处方全样本审核及审方标准同质化，将区域处方合格率提升至96%。
多年来，药剂科通过人才引进、自主培养等多途径加强药学人才队伍建设及药学学科能力建设，加强科教研同步发展。近五年引进高级人才1人，培养松江区领军人才1人，区卫健委医苑新星1人，自主招聘硕士研究生9人。2012—2019年连续8年承担国家级继续医学教育项目。2016年获批上海市首批社区临床药师培训基地，目前已培养71人，2016年、2021年2次荣获“优秀临床药师培训基地”，近5年来承担上海市自然科学基金、松江区科技攻关项目、上海市科委项目9项，发表SCI论文7篇，核心期刊论文40篇。2023年我院正式成立松江研究院，药剂科将积极对接研究院，以期未来药学学科高水平高质量发展。
Department of Pharmacy，Songjiang Hospital Affiliated to Shanghai Jiao Tong University School of Medicine
The Pharmacy Department of Songjiang Hospital Affiliated with Shanghai Jiao Tong University School of Medicine, operates as an integrated technical department that encompasses drug supply and management, clinical medication guidance and supervision, as well as the teaching and training of community clinical pharmacists. It has established a management model centered on patient medication safety and is dedicated to the rational use of medication in its clinical pharmacy endeavors. The department is composed of nine distinct sections: the Clinical Pharmacy Office, Outpatient and Emergency Western Pharmacy, Chinese Pharmacy, Infectious Disease Pharmacy, Fever Pharmacy, Inpatient Pharmacy, Operating Room Pharmacy, Pharmacy Intravenous Admixture Services (PIVAS), and the Pharmacy Warehouse. The department is staffed by 70 professionals, including 3 Chief Pharmacists, 2 Deputy Chief Pharmacists, and 45 Senior Pharmacists. The team is complemented by 1 Doctor, 9 individuals with Master's degrees, and 2 Master's Tutors. In 2019, the department was awarded the Songjiang District Medical Key Discipline A construction project, as well as the hospital-level Key Discipline B. Subsequently, in 2021, it was officially recognized as the Shanghai Key Specialty in Clinical Pharmacy.
In recent years, the Pharmacy Department has continuously promoted the informatization of hospital pharmacy management, and has introduced advanced equipment such as automatic drug dispensers, intelligent anesthesia cabinets, logistics robots, inpatient drug sorting machines, and intravenous drug dispensing robots, gradually establishing a modern drug management mode. On the basis of ensuring drug supply, the pharmacy department actively carries out pharmacy service projects, and currently has 14 full-time clinical pharmacists, implements the responsibility system of clinical pharmacists' wards, and participates in clinical diagnosis and treatment through activities such as clinical rounds, case discussions, and drug education. In 2018, the 'Reasonable Use of Drugs Innovation Workshop' led by Chen Jian, the former director of the Pharmacy Department at Songjiang Central Hospital, was recognized by the Shanghai Municipal Federation of Trade Unions as the 'Rational Use of Medicines Innovation Workshop', and proactively advanced services to meet outpatients' needs for knowledge on rational drug use. In December 2019, the pharmacy clinic was opened, and gene testing technology was utilized to carry out genetic testing for warfarin, clopidogrel, and statin drugs, providing individualized medication services for patients. In 2022, we plan to enhance the specialty construction of the pharmacy outpatient clinic, which includes several subspecialty clinics such as the precision pharmacy clinic, anticoagulation pharmacy clinic, anti-infective pharmacy clinic, anti-tumor pharmacy clinic, and gynecological and pediatric pharmacy clinic. In 2019, our hospital officially passed the inspection for the drug clinical trial base and set up the office of the drug clinical trial organization in the Pharmacy Department, which was equipped with a dedicated site and sufficient facilities and equipment. The hospital has completed the filing of three specialty groups in respiratory medicine, cardiovascular medicine, and emergency medicine, and has completed three drug clinical trial projects, with another three projects in the stage of project establishment.
As a regional medical center, the Department of Pharmacy actively enhances its pharmacy's influence and service reach, promotes the quality and capacity of pharmacy services within the medical community, and is gradually forming a regional integrated pharmacy service center. Since 2016, the Department of Pharmacy has established a partnership support mechanism with 18 community health centers to jointly enhance pharmacy management and services. In 2020, the Department of Pharmacy, as the primary driving force, has promoted the establishment of the regional prescription center project in Songjiang, achieving uniform standards for regional prescription full sample examination and prescription, and raising the regional prescription qualification rate to 96%.
Over the years, the Department of Pharmacy has strengthened the development of its pharmacy talent team and the capacity of the pharmacy discipline through talent recruitment, independent training, and other methods to enhance the synchronous development of science, education, and research. In the past five years, the department has recruited one senior talent, and trained one Songjiang District Leading Talent and one Rising Star of the District Health Committee, and has independently recruited nine master's degree graduates. From 2012 to 2019, the department conducted the national continuing medical education program for eight consecutive years, and in 2016, it was approved as one of the first community-based clinical pharmacist training bases in Shanghai, training a total of 71 individuals and being awarded 'Excellent Clinical Pharmacist Training Base' twice, in 2016 and 2021. In the past five years, we have undertaken nine projects funded by the Shanghai Natural Science Foundation, the Songjiang District Science and Technology Research Project, and the Shanghai Science and Technology Commission, publishing seven SCI-indexed papers and forty papers in core journals. In 2023, our hospital will formally establish the Songjiang Research Institute, and the Department of Pharmacy will actively support the research institute to achieve high-level and high-quality development of the pharmacy discipline in the future.
