临床药师培训基地

上海市胸科医院药剂科下设门急诊药房、中药房、药库、住院药房、静脉药物配置中心、临床药学室六个部门，承担全院药品质量管理、临床药学实践、药学科研等药学相关工作。其中临床药学室成立于2007年，现有专职临床药师11人，包括副主任药师2名，主管药师3名，其中博士学历5人，硕士学历6人。

临床药师中，1人为美国PharmD教育背景。4人完成了临床药师的师资培训，其中3人已获得中华医学会临床药学分会师资培训证书（重症医学、呼吸内科，肿瘤科），1人为中国医院协会带教药师（抗感染）。另有5人完成临床药师规范化培训并进行临床工作，包括中药临床药师1人。人员储备能够较好的满足带教需求。临床药学室已完成信息化建设，已配备并应用前置审方系统、临床药师工作站以及基于定量药理的精准给药等信息化系统。信息化系统满足学员规范化培训要求。

临床药学室多年来以合理用药为核心，开展临床药师全天候“驻”临床工作。已形成规范化临床药师工作体制，配备了专科临床药师参与疾病诊断治疗、用药方案制订和评价，提高了临床用药的安全性。目前已在抗感染和移植，危重症，肿瘤，肿瘤术后康复，心血管和抗凝，呼吸慢病，胸外科等安排驻科临床药师进行全程化的临床药学服务。同时在肿瘤药学门诊，心血管外科门诊，血栓疾病联合门诊，肺移植药学门诊，术后呼吸功能康复门诊和TDM联合门诊针对门诊患者，开展了线上和线下（云服务）的门诊临床药学服务。

临床药学室近年来着重发展与临床相结合的科研工作。2019年至今已获得国家自然科学基金课题3项，其中青年项目2项，面上1项。此外，教学方面承担了交通大学医学院、交通大学药学院博士和硕士研究生的培养工作，于2022年10月取得交通大学药学院教育教学实践基地，承担本科生校外见习工作。

The Pharmacy Department of Shanghai Chest Hospital is divided into six sections: outpatient pharmacy, traditional Chinese medicine pharmacy, pharmacy warehouse, inpatient pharmacy,pharmacyintravenousadmixture services, and clinical pharmacy department. It is responsible for the quality management of all hospital medicines, clinical pharmacy practice, and pharmaceutical research and other pharmacy-related work. The clinical pharmacy department was established in 2007 and currently haselevenfull-time clinical pharmacists, includingtwo associatechief pharmacists andthreepharmacists-in-charge, withfive pharmacistsholding adoctor’sdegree andsixholding a master's degree.

Among the clinical pharmacists,one pharmacisthas an educational backgroundofthe PharmD programin the United States.Four pharmacistshave completed the training for clinical pharmacypreceptors, ofwhom threehave obtained thepreceptortraining certificate from the Clinical Pharmacy Branch of the Chinese Medical Association (intensive care medicine, respiratory medicine, oncology), andone pharmacististheteaching pharmacist from the China Hospital Association (infectious disease). In addition,five pharmacistshave completed standardized training as clinical pharmacists and are engaged in clinical work, includingonespecialized in traditional Chinese medicine. The personnel reserve can well meet the teaching requirements. The clinical pharmacy department has completed the construction of information systems, equipped and applied prescriptionpre-verificationsystems, clinical pharmacist workstations, and precise dosing systems based on quantitative pharmacology. The information system meets the standardized training requirements for trainees.

For many years, the clinical pharmacy department has taken rational drug use as its core and carried out 24/7 "stationing" of clinical pharmacists in clinical work. A standardized clinical pharmacist work system has been established, with specialized clinical pharmacists participating in disease diagnosis and treatment, formulation and evaluation of medication plans, and improving the safety of clinical medication. Currently, resident clinical pharmacists have been arranged in anti-infection and transplantation, critical care, oncology, postoperative rehabilitation of oncology, cardiovascular and anticoagulation, respiratory chronic diseases, and thoracic surgery to provide a full range of clinical pharmacy services. At the same time, online and offline (cloud service) outpatient clinical pharmacy services have been carried out for outpatients in oncology pharmacy clinics, cardiovascular surgery clinics, thrombotic disease joint clinics, lung transplant pharmacy clinics, postoperative respiratory function recovery clinics, and TDM joint clinics.

In recent years, the clinical pharmacy department has focused on developing scientific research work combined with clinical practice. Since 2019, it has obtainedthreeprojects from the National Natural Science Foundation of China. In addition, in terms of teaching, it has undertaken the training of doctoral and master's students from the School of Medicine and the School of Pharmacy of Shanghai Jiao Tong University, and in October 2022, it became a teaching and education practice base of the School of Pharmacy of Shanghai Jiao Tong University, undertaking the off-campus internship work for undergraduate students.
