消化科由我国著名的消化病学专家巫协宁教授始创于1985年,经过半个世纪几代消化科医生的艰苦努力,学科发展进入新的历史阶段。
消化科为国家卫生部消化内镜培训基地、上海市胰腺疾病重点实验室、上海交通大学医学院胰腺疾病研究所、上海交通大学硕士、博士学位授予点和博士后流动站,卫生部消化内镜培训中心、卫生部住院医师和消化专科医师规范化培训基地、国家药物临床试验机构、上海市胰腺疾病研究重点实验室、上海交通大学炎症性肠病诊治中心所在单位,也是中华医学会消化病学分会常务委员、中华消化病学会胰腺疾病专业学组组长、中华医学会肝病学分会副主任委员、上海市消化病学会主任委员和上海市肝病学会副主任委员所在单位。
目前消化科共有医师45人,其中博士学历41人,硕士学历4人。正高职称7人,副高职称11人。博士生导师5名,硕士生导师8名。科室核定床位共120张。设有门诊(消化专科门诊、专家门诊、特需门诊、脂肪肝门诊、胃食管反流门诊、胆胰门诊、便秘门诊和炎症性肠病门诊)、内镜中心、HP室、消化疾病研究室等特色专科医疗。
消化科目前分设胃肠疾病、胆胰疾病、肝病以及消化内镜诊治等亚专业,拥有先进的诊疗及科研设备,能开展各项专科项目并形成如下医疗特色:1.胰腺疾病诊治,特别是重症胰腺炎综合治疗、慢性胰腺炎内镜治疗、胰腺癌早期诊断以及胰腺癌非手术治疗均居国内领先。2.消化系疾病介入治疗肝癌、胰腺癌、胆管癌、食管癌等消化系肿瘤,胆管、胰管、食管和胃肠狭窄及胆道、胰管结石。3.炎症性肠病的中西医结合治疗。4.慢性乙型和丙型病毒性肝炎的抗病毒治疗、肝纤维化和脂肪性肝病早期诊断与发病机理研究、自身免疫性肝病诊治、肝硬化及其并发症的诊治。这些医疗特色和方向均为国内先进水平,吸引了大量本地和外地患者。
目前,消化科的研究方向主要有:胰腺疾病的临床诊治及基础研究、肠屏障功能障碍在重症急性胰腺炎发病中的作用、内镜诊疗新技术研究、慢性肝病的早期诊断和评估及其并发症的诊治、炎症性肠病的诊治及消化道肿瘤的早期诊断和治疗等。形成了胰腺和胆道疾病、胃肠疾病、消化内镜诊治和慢性肝病的学科特色。
消化内镜中心在常规胃肠镜基础上继续发展ERCP、超声内镜(EUS)、内镜下大块粘膜剥离切除术(ESD)、小肠镜、胶囊内镜等技术项目。开展国内领先新技术项目--“超声胃镜下经胃胰腺脓肿穿刺引流术”,“POEM”及胆道碎石等技术。

Introduction
Gastroenterology as one of our characteristic disciplines has a long history and excellent cultural inheritance. It was founded in 1985 by Professor Wu Xiening, a well-known domestic specialist in gastroenterology. Through hard work of several generations of gastroenterologists for half a century, the discipline has reached a new level. Presently, the Department totally has 45 physicians including 41 doctors with MD & PhD degree and 4 with master degree, 7 persons with senior professional title and 11 persons with associate senior professional title. There are five Doctoral Supervisors and eight Master's Supervisors in the department. There are 120 approved beds. Academic leader of Gastroenterology is Professor Wang Xingpeng. Director and Chief Physician is Professor Lu Lungen. Executive Director of Northern Branch is Professor Wan Rong, Deputy Executive Director of Northern Branch is Associate Professor Cai Xiaobo, Deputy Director of Southern Branch is Professor Professor Zeng Yue and Li Baiwen.
Nowadays, the Department has become the place of awarding the master and doctor's degree, the moving station of postdoctors of Shanghai Jiao Tong University, Endoscopy Training Center of the Ministry of Health, Standardized Training Base for Resident Doctors and Gastroenterologists of the Ministry of Health, National Clinical Drug Test Organization, Shanghai Key Lab for Pancreatic Disease Study and location of Inflammatory Bowl Disease Diagnosis and Treatment Center of Shanghai Jiao Tong University. There are Standing Committee Member of Gastroenterology Branch of Chinese Medical Association, Leader of Specialized Pancreatic Disease Group of Chinese Society of Gastroenterology, Vice Chairman of Hepatology Branch of Chinese Medical Association, Chairman of Shanghai Society of Gastroenterology and Vice Chairman of Shanghai Society of Hepatology.
There are Outpatients (Specialist Outpatient, Expert Outpatient, Special Outpatient, Fatty Liver Outpatient, Gastroesophageal Reflux Outpatient, Biliary and Pancreatic Outpatient, Constipation Outpatient, Inflammatory Bowel Disease Outpatient), Endoscopy Center, Digestive Disease Lab and other characteristic specialized medical service.
Under the leadership of the senior gastroenterologists Professor Wu Xiening and Wang Guoliang, an academic team primarily including Professor Wang Xingpeng, Lu Lungen, Wan Xinjian and Wan Rong, which has powerful technical force, excellent professional skill and fine style of work has been gradually formed.

The Department now has such subspecialties as gastrointestinal disease, biliary and pancreatic disease, liver disease and endoscopy center. With advanced equipment for diagnosis, treatment and scientific research, it is able to carry out all specialized programs and form the following medical features:
1. Diagnosis and treatment of pancreatic diseases, especially the comprehensive treatment of severe pancreatitis, endoscopic therapy for chronic pancreatitis, early diagnosis of pancreatic cancer and non-surgical treatment of pancreatic cancer, maintains at a domestic leading level.
2. Interventional treatment of digestive diseases including liver cancer, pancreatic cancer, biliary duct cancer, esophagus cancer, stenosis of biliary duct, pancreatic duct, esophagus and gastrointestinal tract, calculus of biliary duct and pancreatic duct.
3. Comprehensive diagnosis and treatment of HP related diseases.
4. Treatment of inflammatory bowel disease by combining traditional Chinese and western medicine.
5. Antiviral therapy of chronic Hepatitis B and C viral hepatitis, early diagnosis of hepatic fibrosis, fatty liver disease and pathogenesis research, diagnosis and treatment of autoimmune liver disease, diagnosis and treatment of liver cirrhosis and its complications. These medical features and directions at a domestic leading level attract a large number of patients from different regions.
The Endoscope Room continues to develop ERCP, EUS, ESD, enteroscope, capsule endoscopy and other technical items based on the regular gastrointestinal endoscope. It conducted the domestic leading technical item "pancreatic abscess puncture and drainage through stomach under ultrasonic gastroscope", "POEM" and lithotripsy for biliary duct, which fills the gap. Annual outpatient visit, quantity of inpatients, the number of patients accepting endoscopic inspection and treatment rank top among all general hospitals of Shanghai.
The present research directions of the Department of Gastroenterology include clinical diagnosis, treatment and basic research of pancreatic diseases, the impact of intestinal barrier dysfunction on causing severe acute pancreatitis , study on new technology of endoscopic diagnosis & treatment, early diagnosis and evaluation of chronic liver diseases and diagnosis & treatment of their complications, diagnosis & treatment of inflammatory bowel disease, early diagnosis & treatment of gastrointestinal tumors. The disciplinary feature of pancreatic and biliary duct diseases, gastrointestinal diseases, digestive endoscope diagnosis & treatment and chronic liver diseases.

In recent days, the Specialized Pancreatic Disease Committee of Chinese Medical Doctor Association was officially founded. President Wang Xingpeng of our hospital was employed as the Vice Chairman of the first committee.
Pancreas as one of the most important organs of human body has the endocrine function. However, it's very hard to be clinically diagnosed and treated due to its deep position and unclear pathogenesis. Acute pancreatitis is a common disease of the internal medicine. It also occurs with multiple organ failure, which is characterized by serious condition, rapid change, difficulty in treatment and high death rate. Chronic pancreatitis, irreversible, always occurs with intractable pain, serious malnutrition and diabetes. As the lesion prior to pancreatic cancer, it needs to be treated with medicine for life. Pancreatic cancer, called "king of cancers", has a survival rate of less than 5% in 5 years.
We are always characteristic in the field of pancreatic disease. The Clinical Medical Center for Digestive Disease of our hospital is now Shanghai Key Lab for Pancreatic Disease Study and Pancreatic Disease Institute of Shanghai Jiao Tong University. There are Standing Committee Member of Digestive Disease Branch of Chinese Medical Association and leader of Pancreatology Group. In addition, the Pancreatic Cancer Diagnosis and Treatment Center of Shanghai Jiao Tong University is attached to it. The Clinical Medical Center for Oncology is the Chairman Unit of Expert Committee for Pancreatic Cancer of CSCO and took charge of compiling the Consensus of Chinese Experts on Diagnosis and Treatment of Pancreatic Cancer of version 2014.

Pay Attention to Gastrointestinal Diseases in Case of Backache

Over the past year, Mr. Du always felt backache whether keeping standing up or sitting down. In addition, there is also sour regurgitation and belching. He was told this may be caused by kidney deficiency, which however was denied upon inspection at the hospital. There was nothing wrong with the waist or back bone and soft tissue. He suffered a lot from the backache.
Doctor Lu Lungen believed that backache may be caused by many reasons. Generally, it will occur when any organization structure of the spine is abnormal. The most common one is protrusion of intervertebral disc which pressures the nerve and causes pain. Furthermore, the soft tissue injury caused by exercise such as muscle sprain and ligament injury may also produce backache. Other causes include degenerative arthritis, ankylosing spondylitis, spinal cord duct stenosis, spondylolysis, scoliosis, different legs length and incorrect posture.
Moreover, it is worth noting that digestive diseases may also cause pain in the back. Sometimes it is even the first symptom, especially when there is nothing wrong with the waist and back bone or soft tissue.
Acute backache may also occur when someone suffers from acute cholecystitis, acute pancreatitis, and anabrosis of duodenal bulb or its back or back wall perforation. Therefore, the expert advises that we should go to the Department of Gastroenterology for inspection in case of backache together with the following symptoms.
I. Recurrent distending pain in epigastrium, sour regurgitation and belching. This is very likely to be peptic ulcer, especially deep anabrosis at stomach or duodenum back wall. The patient with gastric ulcer always feels painful after the meal, while that with duodenal bulbar ulcer often has pain with an empty stomach or at night.
II. Distending pain in upper right abdomen, nausea and vomiting. This is very common for patients with acute and chronic cholecystitis who has right backache which even spreads to the right shoulder when they take greasy food.
III. Emaciation, diarrhea and icterus. In this case, we should pay attention to chronic pancreatitis, gallbladder carcinoma and pancreatic cancer. Pancreatic pain always occurs in the upper abdomen, waist and back, presented in zonesthesia. The pain which is continuous varies in degree.
IV. Distending pain in hypogastrium, borborygmus, change of defecation habit and abnormal excrement in shape and property. This may be intestines problem, such as inflammatory bowel disease and tumor of intestine. The pain may also spread to the waist and back. The patients with intestinal diseases may feel painful in different part.
As common and frequently-occurring diseases, the overall morbidity of gastroenteropathy accounts for about 20% of the population. The older the higher. It is particularly common for the middle aged and elderly people above 50 years old and the male suffers more than the female. Gastrointestinal diseases are always deemed as stubborn illnesses by doctors. We should timely get treatment once we get them so as to control or cure them.
